"One Stage Adolescent Gynecomastia Surgery”

Sacramento, CA-Dr. Delgado has been doing gynecomastia surgery since 1988; he has found that men with extreme gynecomastia will have minimal scarring when he does their surgery in 2 separate stages.  He removes breast tissue and fat in the first stage as seen in the picture below. The first stage will leave a considerable amount of saggy skin (see second picture). Most men choose to have the second stage done to get the ultimate result.

Stage two is usually done as in the office under local anesthesia, once the patient has healed from the first procedure, usually within 5 to 6 months. During the stage 2, Dr. Delgado will remove skin around the areola in the shape of a donut. He then closes the incision with what is referred to as purse string sutures. See the 3rd picture below after only two weeks from surgery!

It is possible for the adolescent young man with extreme gynecomastia to have male breast reduction surgery done in one stage. This is because when young, the elasticity of the skin is such that it may shrink back on its own without the need for a second stage. See the before and after pictures below of a 15-year old boy who parents brought him to Marin County to have surgery with Dr. Delgado even though they had to travel from the East Coast. They have been so happy with the results of their son’s surgery that recently they sent a photo (see below) one year after his surgery.

Consideration needs to be given to the possibility that if a young man is still going through puberty, his male breasts may continue to grow. However, Dr. Delgado states that in all his years of performing gynecomastia surgery, there has not been a need for any patients to need additional surgery.
