During adolescence when young men are going through hormonal changes, many develop breast buds, some more than others. This excess tissue causes the nipples to protrude in a cone shape fashion, giving a “puffy nipple” appearance.

In most cases the developed breast bud, which is considered gynecomastia, will absolve within a few months but it may take as long as a couple of years. If puffy nipples have been present for more than two years, it is very possible that it is permanent. At this point, the only way to achieve a normal looking chest will be by surgery.

Puffy nipples are caused by excess breast tissue and not a lot of fat. Therefore, liposuction alone will only remove fat, leaving the breast tissue behind making the nipples even more pronounced.

The recommended surgery for “puffy nipples” is the same as for “true” gynecomastia; using a combination of liposuction and surgical excision of breast tissue. However, the reduction is just in the areola and nipple area, making for a shorter surgery time and a faster healing time.

For some men, the “pull through” procedure may be an option. The “pull through” procedure is a minimally invasive surgical procedure that requires only a small incision 3-5 mm. The cannula for liposuction can fit through this small incision for fat removal, and then a special instrument is inserted to remove the breast tissue.
